- Care management, planning and coordination
- Medication administration, monitoring and other skilled treatments/services
- Monitoring changes in health condition and notification of family/physicians
- Ongoing clinical assessment
- Developing personalized care plans in collaboration with client and/or loved ones
- Supervision of home health aides and personal care aides
Each client’s specific needs will determine whether a PCA, a HHA or a Nurse (or combination thereof) should be utilized.
